Summary Measure of Social Determinants of Health/Health Equity and Cardiovascular Disease Outcomes among Mississippi Adults

Abstract Body: Background
Cardiovascular disease (CVD) is the leading cause of death in Mississippi. Mississippi adults experiencing numerous social determinants of health/health equity (SDOH/HE) risk factors have increased odds of CVD than those experiencing no SDOH/HE risk factors. We examined the association between a summary measure of SDOH/HE and CVD outcomes among Mississippi adults.
Methods
Using the SDOH/HE Module data from 3,867 respondents who self-identified as non-Hispanic Black or White in the 2023 Mississippi Behavioral Risk Factor Surveillance System survey, we conducted multivariable logistic regression models to examine the association between a SDOH/HE summary measure and CVD outcomes.
Results
Myocardial infarction/heart attack and angina or coronary heart disease (CHD) prevalence was 4.4% and 5.0% respectively, and the likelihood of experiencing 0, 1, 2, 3, or 4 or more SDOH/HE risk factors was 38.6%, 22.9%, 13.7%, 7.9%, and 16.8% respectively.
Mississippians experiencing four or more SDOH/HE risk factors had higher odds of heart attack or myocardial infarction (AOR, 4.83, 95% CI, 2.15%–10.88%) and angina or CHD (AOR, 2.51, 95% CI, 1.04%–6.09% ) compared to those experiencing no SDOH/HE risk factors respectively.
Conclusions
Our results suggest that Mississippi adults with multiple SDOH/HE risk factors have significantly higher odds of myocardial infarction/heart attack and angina/CHD than those with no SDOH/HE risk factors. These findings highlight the importance of addressing multiple SDOH/HE factors of CVD outcomes. Targeted interventions to reduce CVD burden among Mississippi adults should prioritize improving health-related social needs.
