Coronary Intravascular Lithotripsy for severely calcified lesions: Insights from the disrupt CAD trials

Coronary calcifications pose significant difficulties during PCI procedures such as stent placement and adversely affect the procedural success rate. Shock wave intravascular lithotripsy is a novel technique that uses acoustic pressure waves to disrupt heavily calcified coronary lesions with minimal complications. Thus, we aim to explore the safety and efficacy of IVL treatment in improving procedural outcomes.

Methods
We did a systemic review and meta-analysis of a total of 664 patients involved in the Disrupt CAD trials. The primary endpoint was procedural success rate and secondary endpoints were in-hospital complications and 12 months follow-ups including MACE.

Results
All the studies showed high procedural success rates with a weighted average of 94%. The in-hospital complication rates were 6.9%. The 12 months follow up outcome of MACE was 15.2% with a Target Vessel Revascularization rate of 12%.

Conclusion
Coronary IVL can be an effective tool for stent implantation despite greater target lesion complexity and provides excellent stent expansion. Long-term follow-up is required for a better understanding of the merits of stent implantation using IVL to boost treatment success.
